Admissions

REGISTRATION / ADMISSIONS PROCEDURE

Parents/guardians of all students who have been accepted at the Flatbush Seventh-day Adventist School are to be present to complete child’s registration during the dates set for such registration.

The following are required for the registration procedures:

Complete physical examination form signed by a qualified physician and evidence of immunization.
Present Birth or Baptismal certificate.
Have registration form completed.
Tuition-minimum of 2/10 of the year’s tuition to be paid.
Payment of lab and computer fees where applicable.

THE SIGNING OF THE REGISTRATION FORM IS A CONTRACT INDICATING THAT THE STUDENT AND PARENT PLEDGE TO RESPECT THE PRINCIPLES OF THE FLATBUSH SEVENTH-DAY ADVENTIST CHURCH SCHOOL AND TO OBEY AND SUPPORT ITS REQUIREMENTS AND REGULATIONS.

ADMISSIONS REQUIREMENTS

Five Year-Old Program-Kindergarten

1. Five years of age by December 31.
2. Submission of birth certificate.
3. Submission of official evidence of a physical examination by a licensed physician.
4. Submission of official evidence of immunizations.

Grades One to Five

1. Six years of age by December 31 to enroll in the first grade. Must submit birth certificate. However, if a student has graduated from recognized kindergarten class, will be six (6) years old by January 31 of the following year, and has demonstrated through testing a social and academic readiness for Grade 1, he/she may be admitted to Grade 1. Students will be accepted in Grades 2-5 providing they meet all other admission requirements.
2. Submission of official evidence of a physical examination by a licensed physician for all new students.
3. Submission of request for all previous elementary school records.
4. Immunization required by the state of New York.

FEES FOR REGISTRATION DAY:
1. Registration Fee	Grade K-5- $350.00; after Aug. 3, 2017, $320.00 – NON-REFUNDABLE
2. Re-application Fee:	$50.00 NON-REFUNDABLE (Returning Students)
3. Application Fee:	$75.00 NON-REFUNDABLE (New Students)
4. Home & School Fee:	$20.00 (Per Family)
5. Tuition Payment:	(see Tuition Payment Plan).

1. A discount of 2% will be granted on 2^nd child of anyone having two (2) children in school, or 1% each.

2. A discount of 1/3 of 10% per child will be granted anyone having three (3) or more children in school.

3. A discount of 5% will be allowed for those who pay the entire year’s school fees at the time of registration. A discount of 2% will be given on each additional child’s account.

4. Verification of Seventh-day Adventist Church membership is needed for Seventh-day Adventist rate. (Parents/Guardians only)

Tuition Payment Plans

Any payment done on the site will include a processing fee of 2.9%

PLAN 1 Payment for the entire school year (September to June) at 5% discount is due at time of registration.

Grades 1 to 5	Other Seventh-day Adventists	$3,600.30
Grades 1 to 5	Non-Seventh-day Adventists	$3,800.30

PLAN 2 Payment will be in two installments at 2.5% discount. First payment is due at the time of registration. Second payment is due by January 5. No discount will be given if full payment is not received after registration or the January due date.

PLAN 3 Payment will be in four (4) installments. First payment is due at the time of registration. Second payment is due by November 5. Third payment t is due by January 5. Fourth payment is due by March 5. If second payment is not made by the due date, this plan will automatically be changed to Plan 4.

PLAN 4 Payment will be in ten (10) installments and nine payments. The first two (2) payments are due at the time of registration. The other eight payments area due on or before the 5^th of the following months: October, November, December, January, February, March, April and May.
